With over 600,000 hysterectomies being performed each year in the United States, two thirds of those are for non-cancerous conditions. This means that almost 400,000 hysterectomies are performed each year that are potentially not needed.
This has led to a myriad of treatments for patients with non-cancerous uterine fibroids in an attempt to provide alternatives for women who want to avoid a hysterectomy. However,
In the section below, we outline the most popular treatments and discuss what type of patient is the best fit for each of them. Then we will discuss the FAS technique and what type of patient is the best candidate for this procedure.
While this procedure is less invasive than surgery and recovery time shorter, there are many downsides to this treatment.
The first downside is something your Doctor may not tell you ... this procedure is most effective for single small fibroids.
The second downside is the technique itself. This procedure does NOT remove any fibroids. Blood supply to the tumor is blocked, causing the tumor to shrink and die. This leaves decaying tumor tissue in your uterus, which can produce potential infection and in some cases an urgent need for hysterectomy. Research has also shown an increase in the potential for fibroids to turn cancerous, as a result of degeneration, after UAE. If not taken out ... the patient has no idea what is happening to the tumor.
This procedure is not recommended for women who want to maintain fertility and the ability to have children.
Optimal Candidate
While many doctors claim this procedure can safely remove fibroids while preserving the uterus and fertility, the success rate is based on the size, number and location of tumors.
If you have large or multiple fibroids and or your fibroids are deeply imbedded in the uterus, the chances of complete removal of all your tumors is minimal. Women often experience an "incomplete surgery" where tumors are left behind due to major surgical bleeding.
The next downside is the issue of informed consent. Most doctors will require patients having a myomectomy to sign a consent at the time of surgery for a "possible hysterectomy". This gives your doctor the right, with your consent, to decide for you, whether or not to keep your uterus. Frequently, at the time surgery the extent of the tumors can be much worse than anticipated or what is actually diagnosed on tests. Your doctor can decide at that time to not even attempt a myomectomy, but proceed right to a hysterectomy instead!
While the standard myomectomy can be successful for a few small fibroids the chances of complete removal of all tumors dramatically declines when there are multiple or large fibroids.
Optimal Candidate
Doctors offer this approach "out the gate" as a means to delay surgery. Hormonal therapy can help reduce some symptoms but in truth is only a band aid therapy that has limited effect on fibroid tumors. It is not recommended as a longterm solution.
The most common are the birth control pill, and intra-uterine devices. Both can reduce bleeding in some women but the tumors will continue to grow and spread with time.
Hormonal treatments, like "estrogen agonists", work by blocking the production of both estrogen and progesterone. These drugs can significantly reduce bleeding and can stop cycles completely. It is also known to stop the growth of fibroids. However, once the drug is stopped, the tumors will regrow and symptoms will recur quickly.
The downside of hormonal treatment drugs are the side effects, and it can only safely be used for 6 months. Side effects include hot flashes, depression, joint pain, sleep problems, vaginal dryness and up to 6 percent loss of bone mass.
Optimal Candidate:
Fibroid ablation via this technique uses radiofrequency energy, a combination of electric and magnetic energy, to shrink fibroids, so they no longer produce severe pressure or excessive bleeding.
While this is a minimally invasive procedure, it does not actually remove fibroids and can increase the chance of malignancy.
There is also a greater chance of post procedure infection, as with embolization, if the tumor is not completely absorbed. The technique is not advised for multiple or large fibroids or for women who want to carry a pregnancy.
Optimal Candidate
Ablation is a minimally invasive procedure that uses heat to burn/ablate the uterine lining. This can reduce the menstrual flow significantly and can give women a break from their monthly hemorrhaging. It is often only a temporary band aid for heavy bleeding and does not directly treat fibroid tumors.

The FAS technique is an innovative and highly effective approach to the treatment of fibroid tumor disease. It successfully preserves the uterus no matter the size, number or location of fibroids, and can treat other female diseases like adenomyosis, endometriosis, ovarian cysts and uterine prolapse.
FAS is a bloodless open surgical procedure that uses both vascular surgical techniques, lasers and uterine reconstruction to accomplish the goal of removing fibroids without hysterectomy. We have successfully removed over 500 tumors from one patient as seen in the above video.

At 36 I was diagnosed with uterine fibroids. Because of the large size of them and my desire not to have blood during the procedure, I was told by my physician I would require a hysterectomy because a myomectomy would be too risky.
